Fracture, testicular
a tear of the testicular parenchyma. It is the result of direct blunt injury to the scrotum or straddle injury. Testicular rupture is seen when there are several testicular fractures, associated with a tear in the tunica albuginea. Testicular haematomas and haematocoeles are usually present as well. A testicular fracture usually requires prompt surgical intervention.
Ultrasound is the primary modality in the evaluation of scrotal trauma. A testicular fracture appears as a linear, irregular, hypoechoic intratesticular abnormality and can be mimicked by a tumour, infarct or haematoma. Direct visualization of the actual fracture line is rare. Instead, a heterogeneous testis of irregular contour is seen in cases of testicular rupture. Mixed hypo- and hyperechoic areas correspond to areas of haemorrhage and infarction on pathological examination.
On MRI, the findings of a testicular fracture include lower signal intensity of injured tissue on T2-weighted images when compared with normal testicular tissue. Contrast-enhanced T1-weighted images are particularly sensitive for the detection of testicular fracture. The injured tissues show marked decreased enhancement when compared with the normal tissue. In contrast, testicular tumours tend to show increased enhancement. Small tears in the tunica albuginea are also well visualized on MRI.
